Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees
were exposed to struck
by or crushing injuries.
In the production department, employees used a chain hoist with a damaged
hook to lift and
support the die block and gate of a plastic extruder machine. The tip of
the hook was twisted
approximately 10from the plane of the hook. The die block could fall
causing struck by or
crushing injuries to the employee(s).
Among other methods, one feasible and acceptable method to correct this
hazard is to follow
American National Standard, ASME B30.16.1-1998, Safety Standards for
Overhead Hoists
(Underhung).
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
