On or about 2/15/06, the forklift operator west of the Fabrication Shop
was exposed to being
struck by an overturning forklift because the employer did not require the
use of an operator
restraint device.
Among other methods, one feasible and acceptable abatement method to
correct this hazard as
referenced in ASME B56.1 - 1993 is to used a restraint device.
PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS NOT REQUIRED FOR THIS
VIOLATION.
