Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that the company
did not perform periodic inspections on the 2X line crane and the 2K line
crane:
a.Sterling Pipe & Tube, Inc. located in Toledo, Ohio:  On or about
September 6, 2006,
the employer did not ensure the company performed periodic inspections on
the 2X line
5 ton crane.  The crane was missing an alarm, hook safety latch, pendant
control
was
missing function labels for the control buttons, the pendant control box
was missing a
large section of the housing, and the hoist rope was double reaving on the
hoisting drum.
b.Sterling Pipe & Tube, Inc. located in Toledo, Ohio:  On or about
September 6, 2006,
the employer did not ensure the company performed periodic inspections on
the 2K line
7.5 ton crane.  The crane was equipped with a malfunctioning safety latch
on the hook
and the pendant control was missing function labels for the control
buttons.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure the cranes are inspected at least annually according to ANSI
B30.17-2003
"Overhead and Gantry Cranes (Top Running Bridge, Single Girder, Underhung
Hoist)".
In accordance with 29 CFR 1903.19(c), abatement certification is required
for
this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

the employer did not ensure the 2X line 5 ton crane hook was equipped with
a
functioning safety latch.
b.Sterling Pipe & Tube, Inc. located in Toledo, Ohio:  On or about
September 6, 2006,
the employer did not ensure the 2K line 7.5 ton crane hook was equipped
with a safety
latch.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure the crane hooks are functioning according to to ANSI B30.10-1999,
Hooks.
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

properly
labeled as to the function of the control pendant button. The control
pendant button for
the "north" function was not labeled.
b.Sterling Pipe & Tube, Inc. located in Toledo, Ohio:  On or about
September 6, 2006,
the employer did not ensure the control pendant for the 2K line crane was
properly
labeled as to the function of the control pendant buttons.  The control
pendant buttons
for the "north", "off", and "down" functions were not labeled.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure the crane control pendants are labeled according to ANSI
B30.17-2003 "Overhead
and Gantry Cranes (Top Running Bridge, Single Girder, Underhung Hoist)".
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

electrical shock:

the employer did not ensure the control pendant for the 2X line 5 ton
crane would not
expose exployes to electrical shock.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure the crane pendant does not expose employees to electrical shock
according to
ANSI B30.17-2003 "Overhead and Gantry Cranes (Top Running Bridge, Single
Girder,
Underhung Hoist)".
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
