Sterling Pipe & Tube, Inc. located in Toledo, Ohio: The employer did not ensure the Rafter end weld station guide rollers were guarded. On April 29, 2015, an employee received crushing injuries when his hand and wrist were pulled into the guide rollers.

Sterling Pipe & Tube, Inc. located in Toledo, Ohio: On or about April 29, 2015, the employer did not report an inpatient hospitalization when an employee was admitted to the hospital after receiving crushing injuries when his hand and wrist were pulled into the guide rollers for the Rafter end weld station.
