Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that under the hook lifting devices used with the J-4 jib hoist located in foundry #1, monorail crane used in the furnace room for foundry #1, and the monorail crane used in the furnace room in foundry #2 were not rated as to their capacities: a.Semco, Inc. located in Marion, OH: On or about October 29, 2007, the employer did not ensure the under the hook lifting device used with the J-4 monorail jib hoist located in the furnace room of foundry #1 was labeled as to its capacity. The device is used to move 70 to 80 pound mold weights from the top of the sand molds. b.Semco, Inc. located in Marion, OH: On or about October 29, 2007, the employer did not ensure the crucible carrier suspended by the monorail crane located in the furnace room of foundry #1 was labeled as to its capacity. The device is used to carry approximately 400 pounds of molten metal crucible throughout the furnace room. c.Semco, Inc. located in Marion, OH: On or about October 29, 2007, the employer did not ensure the crucible carrier suspended by the monorail crane located in the furnace room of foundry #2 was labeled as to its capacity. The device is used to carry approximately 400 pounds of molten metal crucible throughout the furnace room. Among other methods, one feasible and acceptable method to correct this hazard is to ensure the under the hook lifting devices are labeled according to ANSI B30.20 (2003) In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.t,

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the J-4 monorail jib hoist pendant control was not provided with functioning strain relief where the control cable entered the hoist housing: a.Semco, Inc. located in Marion, OH: On or about November 6, 2007, the employer did not ensure the J-4 monorail jib hoist pendant control was provided with functioning strain relief where the control cable entered the hoist housing. Among other methods, one feasible and acceptable method to correct this hazard is to ensure the control pendant cable is provided with adequate strain relief according to ASME B30.16-2003. No abatement certification or documentation is required for this item.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the wire rope used on the under hung hoist in foundry #2 was triple reaving: a.Semco, Inc. located in Marion, OH: On or about November 20, 2007, in foundry #2 the overhead underhung hoist's wire rope was triple reaving on the hoist drum. The hoist is used to lift pouring ladles full of molten metal. Among other methods, one feasible and acceptable method to correct this hazard is to ensure hoist ropes are seated properly on the drum prior to operation according to ASME B30.16-2003. No abatement certification or documentation is required for this item.
