were exposed to chemical burns due to a chemical release:
(a)On or about 02/22/06 the butterfly valve for pump DE-HMD-H403-PU4794 was
operating with the valve indicator in the open position even though the
valve indicator
showed the valve in the closed position, as a result of earlier
maintenance work that
misaligned the valve indicator".
Among other methods, one feasible and acceptable method to correct this
hazard is to develop
and implement written procedures for inspection and certification of
valves and valve open and
closed indicators for proper installation and function..
