recognized hazards, that
were likely to cause death or serious physical harm to employees, in that
employees were
exposed to being struck by unsecured loads from company-fabricated lifting
devices which have
failed as a result of over-loading:
On or about September 11, 2006, employees of Metal Improvement Company,
LLC were
using company-fabricated lifting devices to lift parts such as R&W disks.
The lifts were
not load-rated nor were they tested to ensure that they were capable of
safely lifting the
load.  A feasible form of abatement would be to comply with ANSI Standard,
ASME B
30.20  1996, Below-The-Hook Lifting Devices.
Abatement certification is required of this item in accordance with 20 CFR
1903.19(c).
