29 CFR 1910.212(a)(1):Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: a) At the Composite Machining Room, on or about 11/29/2021: A worker was permitted to operate a Bridgeport Mill Drill without a guarded rotating chuck, resulting in amputations. b) At the Machine Shop, on or about 12/2/2021: Workers are permitted to operate a Bridgeport Mill/Drill without a guard to protect against the rotating chuck, exposing the operator to amputation hazards. c) At the Maintenance shop, on or about 11/30/2021: Workers are permitted to operate a Grizzly Mill/Drill without a guard to protect against the rotating chuck, exposing the operator to amputation hazards.
