A.  Employer knew or should have known that three forklift drivers were
driving Nissan
forklifts (4) without wearing a seat belt at the establishment and were
exposed to the hazard
of being thrown out of the forklift and crushed due to not wearing a seat
belt.  A feasible
and useful method to correct the hazard, among other methods, is to
require the use of seat
belts at all times when operating a forklift.
