Carolina:  The employer did not furnish employment and a place of
employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to a struck-by falling load
hazard:
a)Coffing electric hoist near the pillow line: for the employees working
with a
Coffing 1/4 ton overhead electric hoist, the spring-loaded safety latch on
the
hoist's lifting hook was broken and did not operate in the designed manor.
NOTE: An acceptable means of abatement would be to have the hook repaired
or
replaced before further use.
REFERENCE: ASME B30.16-1998, section 16-1.2.9.
b)Coffing electric hoist near the pillow line: for the employees working
with a
Coffing 1/4 ton overhead electric hoist, frequent and regular inspections
were not
performed, to identify problems such as the spring loaded safety latch on
the
hoist's lifting hook was broken and not operating in the designed manor.
NOTE: An acceptable means of abatement would be to begin a frequent and
regular inspection program that provides written documentation.
