Maintenance shop - On or about October 30, 2015, the KBC Tool & Machinery (Type-Grip-1236S) headstock's rotating chuck and lead screw were not guarded to prevent accidental contact with rotating parts during the operation procedure. Instance b. Sorter Area - On or about October 30, 2015, the G+W Mashinen (Type RT330 S/N 260803) conveyor system was not guarded at the ingoing nip points.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Instance a. Maintenance shop - On or about October 30, 2015, the Sunex (model 5002A) 8 inch bench grinder right side wheel's tongue guard was adjusted at 1-inch way from the surface of the grinding wheel. Instance b. Maintenance shop - On or about October 30, 2015, the M30 bilateral bench grinder was missing the tongue guards on both sides of the grinding wheels.
