employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
falling/flying materials and structural
collapse:
a)Northwest side of stadium immediately above the press box enclosure -
Boise State University - 1250
Bronco Circle, Boise, ID;  On or about 4/19/07 and at times prior thereto,
the employer failed to
provide adequate protection for two synthetic slings utilized for hoisting
a concrete beam being removed
from the structure resulting in the slings being cut and the beam dropped,
which exposed employees to
various hazards including but not limited to being struck by
falling/flying materials and structural
collapse.
Note: Abatement certification and documentation is NOT required for this
item.
Note: Abatement of this condition is feasible through compliance with ASME
B30.9c-2000, 'Slings' ,
Subsection 9-4.9(f), 'Operating Practices' requiring "Sharp corners in
contact with the sling should be padded
with protective material of sufficient strength and thickness to minimize
damage to the sling." or, utilize
various products commercially available for sling protection or, correct
the sharp corners/edges of the material
being hoisted or, utilize non-synthetic slings.
