29 CFR 1926.501(b)(1): Employees on walking/working surfaces with an unprotected side and/or edge which is 6 feet or more above a lower level were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: a) On or about May 20, 2021 and at times prior thereto, the employer did not ensure employees were protected while involved in precast concrete activities on the second and third floor of a building being constructed and exposed to fall hazards ranging from 12 feet up to 23 feet.

29 CFR 1926.503(a)(1):A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards: a) On or about May 20, 2021 and at times prior thereto, the employer allowed employees to be exposed to hazardous conditions created by not training each employee potentially exposed to fall hazards. Employees were active in pre-cast concrete work activities and exposed to fall hazards from various heights ranging from 12 feet up to 23 feet above grade.
