a) On or about 8/31/06 in Plant 5 at Hog #1; use of a 4" hand grinder to
redress the metal
blades of the Hog, creating sparks in an area where combustible dust (
cotton dust) was present
without a hot work permit system.
Among other methods, one feasible and acceptable abatement method to
correct this condition
would be the implementation of a written hot work permit system as
described in Section 9.5.2
of NFPA 654 - 2006 edition ( NFPA 51B - 2003 edition).
