that were causing or likely to cause death or serious physical harm to

a) Fabrication Department - In the event of an interruption in the
electrical supply, the Jones
Supreme Machine Company vertical band saw was not equipped with an
operable magnetic
restarter or a similar device to prevent the saw from restarting
automatically when power is
restored.
Among other methods, one feasible and acceptable method to correct this
hazard is to include
but not limited to the following:
Conform to the American National Standards Institute, ANSI B11.10-2003
"Safety
Requirements for Metal Sawing Machines", Section 6.11.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET),
and in addition, documentation demonstrating that abatement is complete
must be
included with your certification.  This documentation may include, but is
not limited to,
evidence of the purchase or repair of the equipment, photographic or video
evidence of
abatement, or other written records.
