Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or before 1/31/2007, at 135 San Lorenzo Avenue, an operator trainee who was operating a tower crane, was exposed to a collapse hazard while the designated person was setting up on the ground. One feasible and acceptable method to correct the hazard of collapse is to provide direct supervision at all times when the trainee is operating, as referenced in American Society of Mechanical Engineers, ASME B30.3-1996 Section 3-3.1(a)(2). PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
