Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees, in that employees were
exposed to the hazard of being caught
between the top of the operators cab of a standup narrow aisle reach fork
truck and the horizontal shelves of
storage racks which can enter the operator's compartment when the truck is
traveling in reverse.
On or about 10/04/06 in the warehouse, and employee operating a Raymond
Electric Standup Reach Fork
truck, model number EASIR 30 TT, was traveling in reverse and struck a
storage rack.  The employee was
crushed between the top of the operators cab and the horizontal member of
the rack.  Other operators
continued to use this model of truck.
Feasible abatement methods include, but are not limited to, the following:
1.  Lower all shelves on the site so that the rear of the cab will strike
the rack and the rack will not intrude
into the cab.
2.  Utilize trucks that have an extended backrest or other raised areas on
the top of  the cab such that the
raised areas will not allow the truck to pass under the rack.
3.  Lower the second tier of the rack so that the truck's existing
overhead guard will strike the rack and not
allow it the truck to pass under the rack.
4.  Purchase trucks that have corner post or other guards to prevent the
under ride from occurring.  These
posts extend from the rear corners of the truck up  to the overhead guard.
