29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following (A) through (D): (A) A specific statement of the intended use of the procedure; (B )Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and, (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. (a) On or about November 20, 2025, the employer did not ensure energy control procedures specifically outlined the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy for machines including, but not limited to, the CA-01 BC700 Hank Cutter. Employees who perform servicing and maintenance tasks are exposed to struck-by, caught-in-between and electrical hazards.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. (a) On or about November 20, 2025, the employer did not conduct a periodic inspection of the companies lockout tagout procedures to ensure procedures were current and correct, and that authorized employees were implementing procedures correctly.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle. (a) On or about October 6, 2025, an employee working on CA-01 BC700 Hank Cutter, was able to access the point of operation through an opening on the interlocked door, part output / discharge shoot and through other openings that did not guard the point of operation. This exposed employees to a caught in and amputation hazard.
