Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The
employer did not
furnish employment and a place of employment  which was free from
recognized hazards
which were likely to cause death or serious physical harm to employees in
that, employees
were exposed to injury while working on rig 557 where the anchor did not
have tags on them
of when they were tested:
a) On or about December 11, 2006, at Hwy 172, Ignacio, CO, Aztec Well
Servicing
Company Rig 557, employees were exposed to injury while working on rig 557
in that, the
anchors did not have tags on them of when they were tested.
Among other methods, one feasible and acceptable abatement method to
correct this
hazardous condition would be to install and inspect all anchors in
accordance with RP4G,
1998 section 7.
