Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The
employer did not
furnish employment and a place of employment  which was free from
recognized hazards
which were likely to cause death or serious physical harm to employees in
that, employees
were exposed to injury while being hoisted with unsuitable winch:
a) On or about December 11, 2006 and at times prior, at Hwy 172, Ignacio,
CO, In the
following instances Aztec Well Service Company, did not ensure employees
were protected
from injury while being hoisted into the air:
1) The air hoist being used to lift personnel was not designed and
approved by
the manufacture for lifting personnel.
2)Employees being lifted by an air hoist were not provided with an
independent
fall protection system.
3)Employees were not provided with a boatswains' chair or an equivalent
lifting
device when being lifted with an air hoist.
Among other methods, one feasible and acceptable abatement method to
correct this
hazardous condition would be to utilize air hoists designed and
manufactured for lifting
personnel. When employees are hoisted ensure fall protection equipment and
use of such
equipment meet ANSI Z359.1-1992,"Safety Requirements for Personal Fall
Arrest Systems,
Subsystems and Components".
Abatement Note:  Abatement certification and documentation is required for
this item
(see enclosed "Sample Abatement-Certification Letter").
