Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to being struck by materials being lifted in the event of a crane failure or by failed structural components of low bay/underslung cranes : A) LOCATION: STOCK CUTTING, LOW BAY CRANE #5685 A DIAGONAL STEEL SUPPORT BEAM USED FOR SUPPORTING ONE END COLUMN OF THE CRANE, WAS BENT AND DAMAGED. IN ADDITION, THE SUPPORT BOLTS USED FOR ANCHORING IT TO THE FLOOR WERE NO LONGER IN PLACE. B) LOCATION: BILLET GRINDING AREA, RUCKLEY BOOTH THE BACK SUPPORT MEMBER USED FOR SUPPORTING A VERTICAL COLUMN ON THE LOW BAY CRANE, #8823, WAS BENT AND HAD NOT BEEN REPAIRED OR REPLACED. C) LOCATION: FORGE WELD THE 2 TON LOW BAY CRANE, #AF1152, HAD A 22 FT LONG CROSS MEMBER USED TO SUPPORT THE RAILS HAD A LARGE BEND IN IT AND THE CROSS MEMBER HAD NOT BEEN REPAIRED OR REPLACED. D) LOCATION: ALONG WALL IN INSPECTION AREA THE REMAINING STRUCTURE FOR AN OLD 1 TON UNDERSLUNG CRANE (SERIAL #137375) THAT HAD BEEN TAKEN OUT OF SERVICE, WAS IN POOR CONDITION WITH BENT PARTS, MISSING ANCHOR BOLTS, AND DEFECTIVE COLUMNS AND THE ENTIRE STRUCTURE WAS NOT TAKEN DOWN OR REPAIRED IN SUCH A MANNER AS TO ENSURE ITS STABILITY AS A STRUCTURAL MEMBER OF THE BUILDING. THE STRUCTURE APPEARED TO HAVE BEEN STRUCK NUMEROUS TIMES BY FORK TRUCKS, AND THERE WAS A DANGER OF HAVING PORTIONS OF FALL IF IT WAS STRUCK AGAIN. E) LOCATION: PROCESS GRIND ACROSS FROM BOOTH #20 TWO COLUMNS FOR THE LOW BAY CRANE WERE DAMAGED AT THE BASE OF THE COLUMNS WITH EACH OF THEM HAVING HOLES THROUGH THE WEB OF THE I BEAM. F) LOCATION: HEAT TREAT NEAR DRILLING AREA THE 2 TON LOW BAY CRANE #1183 HAD ONE COLUMN THAT WAS BENT IN SEVERAL AREAS AND AT THE BASE OF THE COLUMN, THERE WAS A HOLE THROUGH THE WEB OF THE I BEAM. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THESE HAZARDS IS TO ENSURE THAT ALL CRANES ARE PROPERLY INSPECTED AND ENSURE THAT ALL DEFECTIVE COMPONENTS OF THE CRANES BE REPLACED BY THE MANUFACTURES REPLACEMENT PARTS OR REPAIRED SO THAT THEY MEET THE MANUFACTURERS SPECIFICATIONS FOR THE CRANE.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazard of being crushed by large dies and manufactured parts in the event that the unrated and defective structures used to support them failed while supporting these heavy load: A) LOCATION: DIE GRIND AREA DIES WEIGHING UP TO 80 TONS, WERE PLACED ON STEEL FABRICATED "HORSES" THAT HAVE NOT BEEN DESIGNED, TESTED, AND PROPERLY MAINTAINED TO ENSURE THEIR LOADING CAPACITIES. MANY OF THESE "HORSES" HAD SIGNS OF FATIGUE WITH BREAKS AND BENDS IN MANY OF THE COMPONENTS AND THERE WAS NO RATINGS ON THESE SUPPORTS AND NO INFORMATION AVAILABLE TO SHOW WHAT WEIGHTS THEY WERE DESIGNED TO SUPPORT. B) LOCATION: PROCESS GRIND AREA, BOOTH #'S 1,2,10, & 15 STEEL FABRICATED TABLES USED TO HOLD PIECES WEIGHING UP TO 13500 LBS, WERE NOT RATED AND WERE IN POOR CONDITION WITH BROKEN WELDS AND BENT AND CRACKED BRACES MAKING THEIR MAXIMUM SAFE WORKING CAPACITIES UNKNOWN. ONE FEASIBLE AND ACCEPTABLE METHOD TO ABATE THESE HAZARDS IS TO CONSTRUCT NEW SUPPORT STRUCTURES THAT HAVE BEEN DESIGNED AND RATED TO ACCEPT ALL LOADS IMPOSED ON THEM AND INSPECT AND MAINTAIN THEM TO ENSURE THAT THEY CAN SAFELY SUPPORT THE LOADS.
