Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that the
operators are exposed to improperly working equipment:
a) Green Anneal Building - Employees are exposed to finger and hand
crushing/amputation injuries when the lifting pins on the 5000 Ton Press
unexpectedly
fail causing massive steel plates to fall to the bed of the press.
Occurred on 2/9/07
As per ANSI B11.2-1995 Hydraulic Power Presses - Safety Requirements for
Construction, Care, and Use, it is the employer's responsibility to ensure
the
following
4.2.1: The employer shall be responsible for the safety, use and care of
the hydraulic
power press production system.
E4.2.1: The employer's responsibility includes all aspects of the use and
care of the
press production system.  This includes the installation of the press and
its related
equipment, set up and operation of the press and the maintenance of the
complete
system.
Among other methods, one feasible and acceptable means of abatement would
be
to crib or block the steel plates preventing them from falling to the bed
of the
press.
