were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the overhead
hoist, below-the-
hook lifting device or bridge crane system fail:
a)Laser Area, on or about 10/30/07:  The Gorbel bridge crane structure
rated
for 2000 lbs. with a Demag overhead hoist attached which was rated at 2200
lbs. was underrated for what the hoist capacity was rated for.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Prior to installation and use, ensure the supporting structure is
designed to
withstand the loads and forces imposed by the hoist for the rated load in
accordance with ANSI B30.11-1980, Section 11-2.2.2.
b)Laser Area and Plating Area, on or about 10/30/07:  There were no
inspections conducted on a Demag electric overhead hoist and a Ingersoll
Rand air hoist.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Inspect the overhead hoists in accordance with ANSI B30.16-1993, Section
16-
2.1.
c)Laser Area, on or about 10/30/07:  There were no inspections conducted on
an Anver vacuum lift VLS-07.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
1.Inspect the vacuum lift in accordance with ANSI B30.20-1993, Section
20-2.3.
Abatement certification must be submitted for these items.
