4.Re-test and re-rate the jib/bridge structure after modifications have
been made in accordance with ANSI B30.11-1980, Section 11-1.3.5.
Abatement certification must be submitted for items a-1 through a-13, b-5,
c-1, c-2,
c-3, c-4, c-5, c-6, c-8, c-9, c-10, c-11, c-12.

lifting broach cutters, had no rated load capacity markings, no serial
number, no manufacturer's identification and had not been inspected.
13.Dept. 173, B7, on or about 5/20/08:  A below-the-hook lifting device
attached to a 1/2 ton Ingersol Rand hoist at Monorail #42 had no
rated load capacity markings, no manufacturer's identification and
had not been inspected.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to :
1.Inspect the lifting devices in accordance with ASME B30.20-1993,
Section 20-1.3.
2.Mark the rated load capacity on the device along with the
manufacturer's name, serial number and lifter weight, if over 100 lb.
in accordance with ASME B30.20-1993, Section 20-1.2.
b)The following overhead hoists did not have the pendant controls labeled
as
to what they controlled and the wire ropes were damaged and there were no
rated load capacity markings on the hoist:
1.Dept. 890, HH31, on or about 4/16/08:  The pendant control for the
two ton Coffing overhead hoist on Jib 802B at the Federal Broach
#291236 did not have the control buttons labeled as to what they
controlled.
2.Dept. 890, FF29, on or about 4/16/08:  The pendant control for the
one ton Yale overhead hoist on Jib 803 at US Broach #291235 did not
have the control buttons labeled as to what they controlled.
3.Dept. 1453, H34, Battery Charging Area, on or about 4/29/08:  The
wire rope on a Cleveland Tramrail two ton crane had several broken
wires and one section the wires were crushed.
4.Dept. 871, G24, on or about 5/12/08:  A CM Cyclone hand hoist
attached to a Jib Crane #1195 at the Okuma grinder #291693 had no
rated load capacity markings on the hoist.
5.Dept. 871, G24, on or about 5/12/08:  A CM Cyclone hand hoist
attached to a Jib Crane #1194 at the Okuma grinder #291694 had no
rated load capacity markings on the hoist.
6.Dept. 0510, T1, on or about 5/29/08:  An Ingersoll Rand overhead
hoist in Dyno Cell #4 had no rated load capacity markings on the hoist.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Mark the pendant controls as to what they control in accordance with
ANSI B30.16-1993, Section 16-2.3.
2.Replace the rope in accordance with ANSI B30.16-1993, Section 16-
2.4.2.
3.Inspect the overhead hoists in accordance with ANSI B30.16-1993,
Section 16-2.
4.Mark the rated load capacity on the hoist or load block in accordance
with ANSI B30.16-1993, Section 16-1.1.
c)The following bridge crane/jib structures had no rating on the bridge
crane
structure, the end stops were worn and/or not adjusted to prevent damage
to the hoist and/or the supporting structures for the hoists were
underrated
for what the hoist capacity was rated for and/or the bridge crane
structures
had not been re-rated and/or testing after modifications were made:
1.Dept. 860, DD26, on or about 4/17/08:  A bridge crane #1113 with a
1/2 ton ARO air hoist attached was not rated.
2.Dept. 860, EE24, on or about 4/17/08:  The rubber end stops on
Bridge Crane #1108, with a 1/2 ton Ingersoll Rand air hoist attached
were worn causing the bridge to slam into the ends.
3.Dept. 660, L33, Tear Down Repair Area, north of robot cell, on or
about 4/29/08:  No end stops were provided on the Spanco bridgecrane
#1148A, with a 1/2 ton ARO hoist attached to prevent the hoist
and load from hitting the electrical panel.  Also, the bridge had
extensions added to it extending the rail system. No re-rating of the
structure was done and no testing was done to the bridge crane after
modifications were made.
4.Dept. 660, L34, Palletize Area, on or about 4/29/08:  An Ingersoll
Rand Zimmerman hoist rated for 350 lbs. was mounted on a bridge
crane #1151 that was rated for 500 lbs.
5.Dept. 864, AA29, on or about 5/6/08:  A bridge crane with a 1/4 ton
ARO overhead hoist attached had no rated load capacity markings on
the crane.  Also, there were extensions welded onto the rail system.
No re-rating of the structure was done and no testing was done to the
bridge crane after modifications were made.
6.Dept. 871, G28, on or about 5/6/08:  A jib crane #1202 at the Okuma
grinder #291977 with a 1/4 ton CM Cyclone chain hoist attached was
marked 1/2 ton but the manufacturer's rating on the jib stated 485
lbs.
7.Dept. 1454, R30, on or about 5/7/08:  A two bridge crane system each
with a 1/2 ton CM Lodestar overhead hoist attached was not rated for
the entire system in that the bridge cranes shared the same center rail,
one bridge crane was rated for 1000 lbs. and there was no rating on
the other bridge crane.
8.Dept. 681, D19, on or about 5/8/08:  Jib #1342 marked 500 lbs. had
a Ergonomic Systems Inc. ergonomic assist rail being used as the jib
crane with the manufacturer's rating of 250 lbs. marked on it which
had a 1/4 ton Aro hoist attached.
9.Dept. 871, E26, on or about 5/12/08:  A jib crane #815 at the Okuma
Grinder #291028 with a 1/4 ton CM Cyclone chain hoist attached was
marked 1/4 ton but the manufacturer's rating on the jib stated 485
lbs.
10.Dept. 871, J26, on or about 5/13/08:  A jib crane #1201 at the Okuma
grinder #219978 with a 200 lb. CM Cyclone chain hoist attached was
marked 1/2 ton but the manufacturer's rating on the jib stated 485
lbs.
11.Dept. 188, S20, on or about 5/19/08:  A bridge crane at National
Broach #2990727, with a one ton Coffing hoist attached had no rated
load markings on it.
12.Dept. 210, L2, on or about 5/28/08:  A jib crane #809 at the Okuma
Grinder #290972 with a 1/4 ton CM Cyclone chain hoist attached was
marked 1/4 ton but the manufacturer's rating on the jib stated 485
lbs.
13.Dept. 0510, U2, Thermal Shaker Room, on or about 5/29/08:  A CM
Cyclone 1/2 ton hand hoist and a 1/4 ton Aro air hoist were on a
Bridge Crane #1359 that was rated for 1/2 ton.
Among other methods, feasible and acceptable abatement methods to correct
these hazards are to:
1.Mark the rate load markings on the bridge.
