Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts: (a) Toolcat line, for employees operating the 12001-2P, #205A-01458, ALM manufactured lift, modified by the employer, where the employees had exposure to hazards including, but not limited to: the Toolcat rolling off the drive-on ramps, falling off due to inadequate width of the ramps, potential for locks to not engage within safe operating parameters, load rating of lift not established and employees not adequately trained. Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to follow the ANSI ALI/ALIS-2001, ALI/ALCTV-1998, and ANSI ALI/ALOIM-2000 documents as well as to contact the manufacturer prior to the modification of any automotive lift. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Sample Abatement Certification Letter").tion
