P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of being struck by a falling load from tip loading hooks: A) South end of shop - An employee was moving a steel plate by tip loading the hooks of a two leg chain lifting sling, on or about October 27, 2008. Among other methods, one feasible and acceptable means of abatement is to use clamps or other approved devices that are used to lift steel plates properly in the saddle of the hook before operation in accordance with ANSI B30.11-1973, Monorail Systems and Underhung Cranes, Section 11-3.2.2 and
