Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards, burns, smoke inhalation, eye and skin irritation, and hydrogen sulfide inhalation from crude oil tank overfills. The employer does not ensure that multiple crude oil storage tanks (67, 68, 69, 303, and 1449) are operated with a reliable gauge system reporting tank levels and flows to the operator control room. This hazard occurred on or about September 20, 2007, and at times prior thereto, when operators accessing the storage tank areas were not aware of tank level, or input or output flows from the tanks. One feasible means of abatement for this hazard, among others, would be to repair or replace the Varec gauge system of reporting tank levels (with alarm points) and flows to the operator control room.ol

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazards of lacerations and impalement by plastic shrapnel and gravel associated with the potential explosive rupture of PVC pipe used to transport compressed air. The employer does not ensure that piping used to transport compressed air is rated for use. This hazard occurred on or about October 3, 2007, and at times prior thereto, at the Light Oils Operator Building, where the compressed air system installed at the gravel driveway consists of white, schedule 40 polyvinyl chloride (PVC) pipe, a portion of which is exposed aboveground. Employees working in the area are exposed to the hazard of lacerations and impalement. One feasible means of abatement for this hazard, among others, would be the use of piping materials suitable for the transmission of compressed air.ressed

The employer does not ensure that multiple petroleum storage tanks are operated utilizing inspection and testing procedures following recognized and generally accepted good engineering practices (RAGAGEP). This hazard occurred on or about September 20, 2007, and at times prior thereto, where routine in-service inspections were not conducted. One feasible means of abatement for this hazard, among others, is to establish inspection and testing procedures.
