struck by or crushed by a forklift in that the employer did not ensure
that the seat belt provided
on a forklift was used by the operator:
a)cutting department; a forklift operator operating the #25 Komatsu Type LP
forklift, model FG25ST-12, serial #562542A, on August 6, 2007 was not
wearing
the seat belt that was installed on the forklift.
Among other methods, one feasible and acceptable means of abatement is to
require
operators to wear the seat belts installed on the forklifts.
