were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to a fall hazard:
(a) Benbrook Road Jobsite:  On or about 4/25/08, an employee was observed
on a pallet attached to a
Genie Lift all terrain forklift.  The employee was observed riding the
pallet to the ground.
Among others, one feasible and acceptable method to abate this hazard is
to use a platform meeting the
design and use criteria set fourth in the Industrial Truck Standards
Development Foundation (ITSDF)
B56.1-2005 standard.
