29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: Instances including, but not limited to, the following: a) 3020 Faircourt Lane, Cranberry, PA: On or about February 7, 2019, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use at the mudbox area in that no fall protection of any kind was in use, exposing employees to fall hazards of up to at least approximately 25 feet. b) 3020 Faircourt Lane, Cranberry, PA: On or about February 7, 2019, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use on the scaffold bay next to the mudbox area in that only a single cross brace was in place, exposing employees to fall hazards of up to at least approximately 25 feet. c) 3020 Faircourt Lane, Cranberry, PA: On or about February 7, 2019, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use at ladder entry area in that there was a missing midrail section, exposing employees to fall hazards of up to at least approximately 25 feet. d) 3020 Faircourt Lane, Cranberry, PA: On or about February 22, 2019, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were in use on the scaffolding inside the block wall while employees were performing daily job preparation activities such as loading in work areas, exposing employees to fall hazards of up to at least approximately 30 feet.
