Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falls from elevations: On or about 08/14/08, the employer did not ensure that employees working on the "Monkey Board" of Rig #7 were protected from falls, from an elevation of 25 ft, at all times by the use of a guardrail system, safety net system and/or personal fall arrest system. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to: Assure that any employee that is assigned to work from the Monkey Board is provided with the appropriate fall protection and is properly trained on its use. DISCLAIMERS: 1) You are not limited to the abatement methods suggested above. 2) The methods explained are general and may not be effective in all cases. 3) The employer is responsible for selecting and carrying out an appropriate method.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of Falls from Elevations and Thermal Burns: On or about 08/14/08, the employer did not ensure that employees working from the Derrick "Monkey" Board of Rig #7, approx 25 ft above ground, were provided with an emergency escape (Geronimo Line) device in case of fire or well blow-out. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to: Ensure that a "Geronimo" line is installed each time the rig is rigged up, and to train employees on its proper use. DISCLAIMERS: 1) You are not limited to the abatement methods suggested above. 2) The methods explained are general and may not be effective in all cases. 3) The employer is responsible for selecting and carrying out an appropriate method.
