Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to dust explosion, deflagration or other fire hazards:
a)Building B, Woodworking:  Employees were not protected from the hazard
of fire
and/or explosion resulting from the ignition of the contents of the wood
dust
collection systems and subsequent flame and pressure from what would be
transmitted into the plant through the path that the recirculated air
takes. The
systems, as installed, allows air to be recirculated into the workplace
and were
not equipped with a spark detection and suppression system.
ABATEMENT NOTE: Among other methods, one feasible and acceptable means of
abatement
would be to comply with the guidelines outlined in the National Fire
Protection Association
(NFPA) 664 - Standard for the Prevention of Fires and Explosions in Wood
Processing and
Woodworking Facilities (2007 Edition).
b)Building C, Polishing:  Employees who were working in areas served by
metal
dust collection systems were exposed to fire and/or explosion hazards due
to the
design, construction, and location of the systems.  Enclosed systems were
located
inside the building; air was recycled from the dry dust collector into the
building;
and machinery, ducts, and equipment used in and for these systems were not
grounded, bonded, and/or listed for use in this environment.
ABATEMENT NOTE: Among other methods, one feasible and acceptable means of
abatement
would be to follow the criteria set forth in National Fire Protection
Association Standard - NFPA
484  "Standard for Combustible Metals" (2006 Edition).
