Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that the
employer did not ensure that the wheel speed was kept under the maximum
operating speed as
indicated by the rated speed of the abrasive grinding wheel:
a)  Whemco - Ohio Foundry, Inc. - Lima, Ohio:  On or about August 27, 2009
the employer
did not ensure that the 72-inch Roll Grinding Machine (Waldrich Siegen,
Type WS II HK sp x
9.144, Serial 12480) was prevented from exceeding the maximum operating
speed for the
abrasive grinding wheel being used.
ABATEMENT NOTE:  Among other means of abatement, one feasible and
acceptable method
to abate this item is to follow ANSI B7.1-2000, including Section 6.2 -
Spindle Speed, and
Section 7.1 - Standard Speeds, which would ensure that maximum operating
speed
is not
exceeded for the specific wheel being used on the 72-inch Roll Grinding
Machine.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
