OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the uncontrolled flow of molten metal hazards:    a. Whemco - Ohio Foundry, Inc. - Lima, Ohio: On or about February 2, 2018, the employer did not ensure that the #18 ladle was equipped with a means to control the ladle tilt during pouring operations.  Employees that used the ladle to pour molten metal into furnace #3 were exposed to burn injuries from molten metal in that the operator's tilting mechanism was damaged.     b. Whemco - Ohio Foundry, Inc. - Lima, Ohio: On or about February 2, 2018, employees were exposed to burn injuries from molten metal in that a means to prevent unintentional tilting of ladle #18 was not used prior to the ladle being hoisted and transported by crane or when transferred by the transfer car.  The ladle's locking mechanism(s) was not used prior to hoisting and transporting the ladle from the pour bay to the melt deck area and furnace three.
