Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that the
employees were exposed to falling down the elevator shaft or being crushed
by moving elevator.
a)Parking device on the freight elevator at the loading dock.
The freight elevator parking device was overridden by a piece of wood which
allowed the elevator gates to open while the elevator was outside the
landing
zone.  Employees working on the loading dock were exposed to a fall hazard
of
approximately ten feet; on or about 01/16/08.
One feasible and acceptable abatement method, among others, to correct
this hazard is to follow
ASME A 17.3-1996 Safety Code for Existing Elevators. Section 2.7.2
"Elevator Parking
Device" which states that the parking device shall be installed so that
the
device will not permit
opening or unlocking a door when the car is outside the landing zone.
NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED
IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR
DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY
REQUIRED BY 29 CFR 1903.19..
