operating lift trucks, rubber tired back hoe, and excavators without
wearing seat belts:

Skytrak was not equipped
a seatbelt for the operator to use as observed on 1-28-10.
(b)Water Line Project Rt. 80 North of Gilbert:  The operator of the Cat
rubber tired back hoe did
not utilize the seatbelt as determined on 1-28-10.
(c)Water Line Project Rt. 80 North of Gilbert:  The operator of the Cat
312 excavator did not
utilize the seatbelt as determined on 1-28-10.
Among other methods, one feasible and acceptable abatement method to
correct these hazard is
to ensure all equipment with roll over protection has an operable seat
belt and to train
employees and effectively enforce the use of seatbelts when the equipment
is
provided by the
manufacturer.
