On or about, June 27 2016, employees were exposed to a fall hazard up to 13 feet while installing trusses and a suitable fall protection system was not used. b. On or about, June 27 2016, employees were exposed to a fall hazard approximately 8 feet while standing on top of a concrete vault installing trusses and a suitable fall protection system was not used.
