Employees entering the six baghouse compartments to work from the catwalk
while removing
and installing filters were not protected from falls of approximately 23
feet to the concrete floor
below in that the catwalk was not equipped with a standard guardrail
system and when full body
harnesses were worn by the employees the lanyards were single lanyards
requiring the
employees to disconnect and reconnect the lanyard to the tie-off point so
that the lanyard could
travel along with them while moving on the catwalk.

time of
