29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s), 4 feet or more above the adjacent floor or ground level, were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), at the baghouse: Employees entering the six baghouse compartments to work from the catwalk while removing and installing filters were not protected from falls of approximately 23 feet to the concrete floor below in that the catwalk was not equipped with a standard guardrail system and when full body harnesses were worn by the employees the lanyards were single lanyards requiring the employees to disconnect and reconnect the lanyard to the tie-off point so that the lanyard could travel along with them while moving on the catwalk. 29 CFR 1903.19(c)(2) abatement of the above violation was verified at the time of inspection. No certification is required.tion

Employees accessed the baghouse compartments topside when removing old filters and installing new and/or repaired filters. 29 CFR 1903.19(c)(2) abatement of the above violation was verified at the time of inspection. 29 CFR 1910.1028(k)(1)(ii): The employer's exposure measurement records did not include all the information required by 29 CFR 1910.1028(k)(1)(ii)(A) through (D): The employer's records of the air monitoring conducted to measure the employees' exposure to benzene in the Wastewater Treatment Plant did not include all of the required information as follows: a description of the procedure used to determine representative employee exposures; a description of the sampling and analytical methods used; a description of the type of respiratory protective devices worn, if any; and, the name, social security number, and job classification of the employee monitored and all the other employees whose exposure the measurement is intended to represent. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
