29 CFR 1910.23(d)(1)(iii): Flight(s) of stairs with 4 or more risers, less than 44 inches wide and having both sides open were not equipped with one standard stair railing on each side: Employees were exposed to fall hazards from the stairway while entering and exiting the sludge press trailer: a) The right side of the stairway (entering the trailer) was not equipped with a stair rail. b) The stair rail on the left side of the stairway (entering the trailer) was damaged and did not meeting the required height of 30-34 inches. The stair rail also did not have an intermediate rail (mid-rail). 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.

29 CFR 1910.23(a)(9): Floor hole(s) into which persons could not accidentally walk were not protected by securely held covers: A gap measuring approximately 2.5" X 53" located along the fifth level walkway of the Boiler House on the east side was not covered. The gap was created were the walkway ended and the equipment came up through the floor. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
