Ammonia Refrigeration Engine Room:  The stairway connected to the back
designated exit door
had been separated from the building, leaving a 1 foot gap hole between
the building and the
stairway landing.

Ammonia Refrigeration Engine Room: A rusty 55 gallon barrel, which had
been stored on the
stairway landing of the back exit, obstructed the emergency egress route.

above the adjacent
floor or ground level were not guarded by standard railings (or the
equivalent as specified in 29
CFR 1910.23(e)(3)(i) through (v), on all open sides:
Roof by Ammonia Refrigeration Condensors:  At the time of the inspection,
a portion of the
guardrail was missing. The missing portion was between the 2 condensors
which are less than
4 feet from the side of the roof.
C & S Wholesale Grocer was previously cited for a violation of this
Occupational Safety and
Health standard 29 CFR 1910.23(c)(1) which was contained in OSHA
Inspection Number
310189519, Citation Number 02, Item Number 001, issued on 07/28/2008, with
respect to a
workplace located at 500 North Street, Windsor Locks, CT, which became
final order on
04/01/2009.
C & S Wholesale Grocer was previously cited for a violation of this

from from upper surface of top rail to floor, platform, runway, or ramp
level:
Roof by Ammonia Refrigeration Condensors:  At the time of the inspection,
the top rail of a
guardrail on the roof, 22 feet 8 inches above ground, was 47 inches from
the work platform.
