Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: On or about February 20, 2008, in the yard at Pad 3, employee(s) used a Manitowoc 4000W Vicon (A04) crane to move large sections of steel hull from the vessel to the yard and the dead end of the wire rope lifting line was improperly installed at the wedge socket, such as, but not limited to: a.the dead end of the wire rope was secured to the live end; and b.one of the u-bolt clips had the dead end of the wire rope resting in the saddle. Among other methods, one feasible and acceptable abatement method would be to secure the dead end of the wire rope to a length of rotation resistant wire rope or use a double-saddle clip and properly rest the live end of the wire rope in the saddle of the u-bolt clips in accordance with the manufacturer's recommendations and guidelines.
