Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
flour dust explosion, deflagration, or
other fire hazards:
a.On or about February 12, 2009, in the new flour room, the vent on bulk
flour storage bin #5 which was
designed to discharge to the building atmosphere did not have a filter.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to correct
this hazard include following NFPA 61, Standard for the Prevention of
Fires and Dust Explosions in
Agricultural and Food Processing Facilities (2008 Edition), including, but
not limited to:  installing a
filter having a minimum efficiency of 99.9at 10 microns.
b.On or about February 12, 2009, in the basement, the vents on bulk flour
storage bins #2 and 3 which
were designed to discharge to a separate drum and to the building
atmosphere
did not have filters having
a minimum efficiency of 99.9at 10 microns.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to correct
this hazard include following NFPA 61, Standard for the Prevention of
Fires and Dust Explosions in
Agricultural and Food Processing Facilities (2008 Edition), including, but
not limited to:  installing a
filter having a minimum efficiency of 99.9at 10 microns.
c.On or about February 12, 2009, in the new flour room and basement, the
two dust collection systems
(baghouses) did not have a monitoring system to indicate pressure drop
across the filter media.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to correct
this hazard include following NFPA 61, Standard for the Prevention of
Fires and Dust Explosions in
Agricultural and Food Processing Facilities (2008 Edition), including, but
not limited to:  installing a
monitoring device such as a differential pressure gauge to indicate
pressure
drop across the filter media.
d.On or about February 12, 2009, in the new flour room and basement, the
two dust collection systems
(baghouses) which were located inside of the building were not equipped
with deflagration venting or an
explosion suppression system.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to correct
this hazard include following NFPA 61, Standard for the Prevention of
Fires and Dust Explosions in
Agricultural and Food Processing Facilities (2008 Edition), including, but
not limited to:  equipping the
dust collectors with deflagration venting to the outside through straight
ducts not exceeding 20 feet in
length and designed so that the explosion pressures would not rupture the
ductwork or the collectors;
equipping the dust collectors with an explosion suppression system
designed according to NFPA 69; or
relocating the dust collectors outside of the building and protecting them
in accordance with Section 6.3.
e.On or about February 12, 2009, in the new flour room and basement,
deflagration isolation devices
were not installed between the dust collectors (baghouses) and the bulk
flour storage bins to eliminate
explosion or deflagration propagation.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to correct
this hazard include following NFPA 61, Standard for the Prevention of
Fires and Dust Explosions in
Agricultural and Food Processing Facilities (2008 Edition), including, but
not limited to:  installing
deflagration isolation between the dust collectors and the bulk storage
bins.ge
