Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to the hazard of fire and explosion from dusty air being exhausted from Air-Material Separators, such as bag houses, back into buildings where employees are working, without any deflagration suppression systems, such as "slam gates", being in line: a)During recent winters, exhaust air from the Central Vacuum System's dust collector in Building C was being recirculated into Plant C without deflagration suppression systems. b)Throughout the year, double filtered air from a bag house located near the C Blender in Building C was being recirculated into Plant C without deflagration suppression systems. Feasible abatement methods would include, but not necessarily be limited to: (1) Venting exhaust air from bag houses to the outside rather than having it re-enter Building C; or (2) The installation of devices which would prevent transmission of energy from a fire or explosion involving these bag houses from re-entering the building.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment, and a place of employment which were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to the hazards of fire and explosion from Pulverizers which generated combustible dust and did not have explosion venting or deflagration suppression systems in place: a)A Pulverizer located in Building A, and one of the three Pulverizers located in Building B were not equipped with explosion venting or deflagration suppression systems. Feasible abatement methods would include, but not necessarily be limited to: (1) Installation of Deflagration Venting; (2) Installation of Deflagration Suppression Systems in accordance with NFPA 69; (3) Installation of Oxidant Concentration Reduction Systems in accordance with NFPA 69; or (4) Installation of Deflagration Pressure Containment Systems in accordance with NFPA 69.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to the hazard of potential fire and explosion from dust collectors for combustible dust being located inside buildings: (a)At Plant C, four dust collectors ranging in size from 15 cubic feet to 130 cubic feet in internal volume were located inside Building C and lacked explosion venting; (b)At Plant A, six dust collectors ranging in size from 36 cubic feet to 198 cubic feet in internal volume were located inside Building A and lacked explosion venting; (c)At Plant B, eighteen dust collectors ranging in size from 13 cubic feet up to 212 cubic feet were located inside Building B and lacked explosion venting. Feasible abatement methods would include, but not necessarily be limited to: (1) Relocating the dust collectors outside of the buildings; (2) Equip each dust collector with a deflagration vent which is vented to the outside and which has been designed to withstand the effects of deflagration; (3) Installation of Deflagration Suppression Systems in accordance with NFPA 69; or (4) Installation of Deflagration Pressure Containment Systems in accordance with NFPA 69.
