Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to the improper rigging, labeling, and use of scaffold system components resulting in potential fall hazards: A)American National Skyline, Inc. - On or about, 10/08/08, employees were conducting window washing operations from a boatswains chair that was attached to a mule/outrigger. Employees were exposed to a fall of over 40 ft. to the ground below due to the following deficiencies: 1)No competent person was on site. No manufacturers material/information was available on site for the mule/outrigger to enable personnel to determine the correct amount of counterweights. is to ensure that Transportable Counterweighted Ourrigger has the manufacturer information or professional engineer appoval on site in accordance with ANSI/IWCA I-1402001, Section: 17.5.1 Window Cleaning Safety.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to improper counterweights which did not result in a 4 to 1 safety factor resulting in potential fall/tip over hazard: A)American National Skyline, Inc., NFG Building, west side - On or about, 10/08/08, employees were working on a boatswains chair attached to a roof top roll rig that did not have a 4 to 1 safety factor because personnel were incorrectly calculation the amount of counterweights. Among other methods, a feasible and acceptable means of abatement to correct these hazards is to ensure that a 4 to 1 safety factor is achieved with the counterweights and is in accordance with ANSI/IWCA I-1402001, Section 5.7.17 (c), Window Cleaning Safety.g

Neither the roof top roll rig nor the employees were tied off. Among other methods, a feasible and acceptable means of abatement to correct these hazards is to ensure that the roof top roll rig is tied off, not moved while an employee is occupying the boatswains chair, and by following the requirements of ANSI/IWCA I-1402001, Section 5.7.17 (f), Window Cleaning Safety.g

Employee was tied to the roof top roll rig which was not tied off. Among other methods, a feasible and acceptable means of abatement to correct these hazards is to ensure that a safely secured independent fall arrest system is utilized and following the requirements of ANSI/IWCA I-1402001, Section 5.7.6, Window Cleaning Safety.
