29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety: On or about November 9, 2017 at a jobsite located at 4100 Rockside Road, Independence, OH 44131 during window cleaning operations; the employer failed to protect workers from electrical shock hazards. Unqualified employees were exposed to electrical shock hazards while utilizing a water fed extension pole without adequate training for working near energized overhead power lines. The training did not include how employees were to calculate the working distance from the energized overhead power lines based upon the tools used, angles of approach, and configuration of the jobsite.
