Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to unmarked natural gas shut-offs, improper natural gas
connections, and natural
gas hose connectors that were not suitable for the conditions of use:
a)  St. Gobain Container/Second Floor Area Near IS Machine--Master Heater
--- (On or about
Feburary 3, 2010:
1.) The Master natural gas fired heater was connected to the facility's
natural gas piping
system by hose connector that was not designed for regular movement,
vibration, and
potential physical damage. The hose connector was dented in at least one
location.
2.) The end of the hose connector was slipped over the supply side fitting
and was not
attached by positive mechanical means.
3.) The manual gas shut-off that controlled the flow of natural gas from
the facility's
piping system to the Master natural gas fired heater was not labeled.
ABATEMENT NOTE:
Among other methods, feasible and acceptable methods to correct the
conditions include:
1.  Complying with the applicable requirements noted in NFPA 54/ANSI
Z223.1 -
National Fuel Gas Code
2.  Complying with ANSI A10.10 Safety Requirements for Temporary and
Portable
Space Heating Devices
3. Conspicuously label the natural gas shut-off.
4. Acquire and install gas connectors that are rated, listed, and labeled
for
industrial
environments, regular movement, and vibration.  Flexible hoses made of
various
materials are commercially available.
5. Utilize fittings that positively and mechanically connect.
