29 CFR 1910.110(b)(16): Personnel performing operation and/or maintenance on LPG equipment were not properly trained in such functions. *************************************************************** a) Employees filling the LPG fuel cylinders were not properly trained in all safety aspects. They were not trained as to the procedures to take when leaks occur between the cylinder fill valve and the filling hose nozzle. ************************************* b) Employees filling LPG fuel cylinders were not trained to mitigate all the sources which allow the generation and accumulation of static electrical charges.

29 CFR 1910.110(b)(17)(ii): Open flames or other sources of ignition were permitted in pumphouses, container charging rooms, or other similar locations. *************************************************************** a) Static electricity discharge as a source of ignition was not eliminated from the LPG fuel container filling operation. Grounding cables were not attached to the fuel cylinders while undergoing filling. The accumulation of static charge on employees performing the operation was not minimized. *************************************** b) A forklift truck was left running during the LPG fuel container filling operation. The electrical system of the truck and/or the hot metal parts of the engine and exhaust system can be sources of ignition.
