Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of strking stationary objects on the ground
when using the
emergency descent system:
a)  Nabors Drilling USA, Rig 577, Parachute, CO: The employer had not
ensured that the wire rope on the derrick emergency descent device
terminated
to a safe area.  On and prior to April 28th, 2009, the wire rope was
anchored to
the side of a portable metal building that would have allowed the user to
strike
the building prior to landing on the ground.
Among other methods, one feasible and acceptable method to correct this
hazard
would be to anchor the wire rope away from obstructions in accordance with
the
Recommended Practice for Occupational Safety for Oil and Gas Well Drilling
and
Servicing Operations API 54, section 6.10.1

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by unsecured pressurized gas
lines:
a)  Nabors Drilling USA, Rig 577, Parachute, CO:  On and prior to
4/28/2009,
4 two inch rigid lines having gas pressure between 800 and 1600 pounds,
were
not secured to prevent movement in the event of failure.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to follow Recommended Practice 54 published by the
American Petroleum Institute. Paragraph 9.13.3 states that "each section
of a high
pressure rigid line should be secured using appropriate means to prevent
movement should the line fail."
