Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being "struck by" pressurized lines: a) Nabors Drilling USA, Rig 573, Parachute, CO: On and prior to 4/30/2009, 2-two inch rigid gas lines having pressure readings between 2650 and 3850 PSI were not secured to prevent movement in the event of failure. b) On and prior to 4/30/2009, the safety clamp on the 4 inch flexible high pressure mud line going toward the drilling rig was improperly installed. The safety clamp was attached to the rigid steel coupling at the end of the flexible hose. c) On and prior to 4/30/2009, the safety clamp on the high pressure assembly located in the "mud pump" house was improperly installed. The safety clamp was attached to the rigid steel coupling at the end of the flexible hose. Among other methods, one feasible and acceptable abatement method to correct these hazards would be a) stake or place weighted objects over appropriate sections of rigid gas lines and b,c) install safety clamps in accordance with the hose manufacturer markings found on the hose assembly. Typically these markings are placed 8-12 inches inboard of the rigid steel coupling.
