working on and from a jobmade platform attached to the forks of a Lull,

Rough Terrain Forklift elevated approximately 32 feet above the ground
below. The
platform was not approved by the manufacturer of the Lull, was not
properly secured
to the forks of the Lull and the employee was not provided personal fall
protection.
A FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD,
AMONG OTHER MEANS, WOULD BE:
a) PROVIDE A PERSONNEL PLATFORM APPROVED BY THE MANUFACTURER.
b) PROVIDE FALL PROTECTION SYSTEM TO PROTECT THE EMPLOYEE
WHILE WORKING FROM APPROVED PLATFORM.
